West Holt Medical Clinic partners with Chartspan to provide Care Management

West Holt Medical Clinic is excited to announce a partnership with Chartspan to provide care management to WHMC patients enrolled in Medicare.

Care management is a Medicare program that gives you access to care in between your regular office visits. The program’s goal is to keep you healthy, offer you assistance, and prevent any conditions you have from worsening. A care management program reduces unnecessary hospital visits, ER visits, and overall healthcare costs.

The Chartspan program uses nurses and clinicians to help you manage your health needs outside of office visits. These team members will then report updates back to your provider at WHMC to keep them informed.

Patients enrolled in Chartspan’s care management have access to a 24/7 nurse line, customized care plans, and additional assistance with appointments, prescriptions and more!

Costs associated with Chartspan depend on insurance. Normal copays and deductibles apply. While  Chartspan can help you estimate costs, your insurance company can provide you an exact answer.
