Lights of Love Tribute Program

West Holt Memorial Hospital will be illuminated with Christmas lights dedicated by donors in honor or memory of their loved ones.

To dedicate a light:

1. Print and fill out the donation form

2. Make a minimum $5.00 donation per name

3. Mail your donation and lights of love form to:

Lights of Love designations will be on display in the hospital’s glass walkway. Donations will be accepted throughout the holiday season.
