Social Worker/Discharge Planner
West Holt Medical Services is hiring Hospital Social Worker/Discharge Planner! This position will work to make sure patient needs are being met at the social, emotional, and economic levels to help ensure WHMS is caring for the whole person.
This position also assists in discharge planning for hospital patients, including contacting appropriate related services and physicians, working on clinical referrals, and keeping abreast of community resources available.
Additional duties include short-term counseling to patients and families that might be experiencing life-changing challenges, directing activity services for Swing Bed patients, and serving as the Civil Rights Coordinator for WHMS.
A bachelor’s degree in social work or a related field is preferred for this position.
This position will work normal business hours but may be required to work outside the 8-5 timeframe as patient needs may require.
